  • Full identification number or passport number
  • The patient’s name and the name of the principal medical scheme member, or the guarantor in the case of private patients. This contains a minor patient’s ID or passport number.
  • Admission date
  • Your treating doctor’s name and surname, as well as the information for your family doctor.

Each time you log into the Patient Portal system, an OTP number will be issued to your registered cell phone number.
