A patient portal is a secure online platform that allows patients to access personal health information from anywhere with an Internet connection, 24 hours a day, seven days a week.

Patients can access health information by using a secure username and password. The CityMD Patient Platform enables you to see, access, and print your test results.

To have access to your patient portal, you must first register your email with CityMD and become web-enabled.

And we’re always looking for new ways to make your health care easier and more connected.

How Do I Begin?

1. Download the My Summit Health iOS or Android app.

2. Launch the app and press the “Create Account” button.

3. To create your account, just follow the steps.

4. My Summit Health is intended for those above the age of 18.

Patients of Summit Medical Group will continue to use their current patient portal.

Summit Medical Group and CityMD will eventually share a single interface. We will keep you updated regarding changes.
