How Do I Get Into DLSU?

1. Application Form

2. Three (3) pieces of Passport size identical photos

3. Report Card

4. Copy of Report Cards for the past two (2) School Years

5. Copy of Current Report Card e.g. applying to Grade 7: Report Cards Grade 4 and 5, Current Card Grade 6

6. NSO Birth certificate

7. Guidance Transition Certificate -to be accomplished by the Guidance Counselor of the school. 

If the school does not have a Guidance Center, the class adviser or any teacher whom you feel has served as the child’s mentor.
